Enrolment stays on paper on purpose — the forms carry medical, consent and emergency details that belong in a signed document, not a web page.
2. Complete and sign
Fill in every section, initial each page at the bottom right, and gather your documents.
3. Email it back

Parent Information Pack
Four pages covering contact details, our welcome and open-door policy, what we offer, school fees, optional activities, stationery, uniform, school hours and our commitment to you.

The form you complete and return. It captures your child's details, medical information, emergency contacts, authorised collection and the school's terms.

Please send these along with your completed application form:
- ID copies of both parents / guardians
- Copy of your child's unabridged birth certificate
- Copy of the clinic / immunisation card, with immunisations up to date for their age
- Copy of the medical aid card (not required if you are not on a medical aid)
- Emergency contact details for at least two people
It is the parents' responsibility to hand in all relevant documents. If an emergency occurs and the school does not have them, Disney Academy takes no responsibility.
Please note
- Applications are processed in the order received and are subject to available space in each age group.
- If the registration fee is not paid within 3 days of notification we cannot hold your space.
- Should we have no space available, your details are placed on a waiting list and you will be contacted as soon as a space opens.
- Your child will only be released to authorised persons. If an unknown person is sent for pick-up, please inform the Principal and a release code will be issued.
- Please initial each page of the application form at the bottom right corner.

Questions parents ask us most
We care for children from approximately 3 months through to 6 years, which takes them all the way to Grade R. We have separate facilities for babies and toddlers.

Full day runs from 06:30 to 18:00, Monday to Friday. Half day children must be collected no later than 13:00. We are closed on public holidays.
Yes. Breakfast and a cooked lunch are included in both the full day and half day programmes. You are welcome to send additional snacks along if you would like to.
Sometimes, depending on the age group and time of year. Applying early is always worthwhile and there is no obligation attached to sending in your forms.
Fees depend on the programme and any optional activities selected. The full fee schedule is in the Parent Information Pack — or contact us and we will send it through.
A change of clothes, a labelled water bottle, and nappies or wipes if applicable. Details of the stationery pack and uniform are in the information pack.
